Physical and Immunization requirements per the Pennsylvania Department of Health.

• It is required that all 6th grade students have a physical exam completed during their 6th grade school year.

• Additional immunizations of Tdap and Meningitis are required for enterance into the 7th grade. (Many physicians will completed these at the same time as they completed the medicalcheck up for 11 and 12 year olds.)

• It is required that all 11th grade students have a physical exam completed during their 11th grade year.

• A second second meningitis vacination is now required for enterance into the 12th grade.

MANDATED SCHOOL HEALTH SERVICES

Every child of school age attending or who should attend a public or private/non-public school within the Commonwealth must receive the following health services. Private/non-public schools may request these services from their local public school district. Via submission of an annual school health report, the local school district is reimbursed by the Pennsylvania Department of Health for provision of the mandated services.
